Receiving Emails without Form Values

Hi,

I am using this module and when I click the submit button email is dispatched without submitted form values. In Mail I received the data like this way : Label Text

It should display the value.

I'm not sure what is happening, you seem to have two copies of the form in the page one near the top and one at the bottom. The one at the bottom is quite badly broken as there is no action URL. It has the same name as the one at the top but has different contents.

Yes I added form for test purpose. But I had removed it.
Would you please check it again.

The form looks OK from the front-end. Please drag a Debugger action into the On Submit event, then submit the form and post the debug - including the 'dummy emails' results here.

Note: if you are using the Easy Wizard you can turn on Debug on the Others tab.

PS In this line next to the form both apostrophes are wrong and should be removed.
