Database selection

Environment:

Joomla! 2.5.9
Chronoforms 4.0 RC3.5.2
localhost

Issue:
Cannot able to set table for the form. It says table created successfully but tables connected is showing empty. Any ideas?

Have you added a DB Save action and linked it to the table you created?

Does the table exist if you check with PHPMyAdmin?

There are no DB tables connected to this form to configure.

I have attached screenshot, Is it missing anything?
