ChronoForm will not update, email, nor disable

I'd like to first preface by stating that I'm very new to Joomla and to ChronoForms.

My Problem: I was asked to update a small form on our company's website and when I updated it through Edit Form Wizard to add a dropdown (to select a Provider to request an appointment), it no longer sends the request email nor displays the dropdown update I made to the form.

The form appears at the top of my provider directory (made in SOBI2) and on every subsequent provider entry pages (http://www.1800snoring.com/dentist-directory-snoring-and-sleep-apnea.html). I tried to disable the form, but it still shows on the entire directory. I like having a mini version of the form on all of these pages to promote appointment requests, but it must have the dropdown I created to select the provider so we know who to make an appointment with.

Please someone help me!

That url doesn't work, but if some form features stopped working then may be you have changed something ? please double check the email settings, you can find many notes about this on our FAQs page.
