Hi guys,
I'm kindof new to this component, and I'm really hoping that this is possible.
Essentially my client wants a regular form on his website, but when a checkbox is selected, he wants 3 more fields to appear for the person to complete.

How do I do this?

Please can you let me know if this is possible or not.

Yes, it's possible. Please see this FAQ for the code to have this work with a single input - it needs to be adapted to work with several inputs.
