Forms work fine and will have all the content emailed from the form. Then randomly I start getting form results like this:

First Name {input_text_0}
Last Name {input_text_1}
New Patient {input_select_10}
Phone Number {input_text_2}
Email Address {input_text_3}
Insurance Company {input_text_4}
Select Date {input_datetime_12}
Desired Appointment Time {input_select_5}
Desired Location {input_select_7}
Preferred Doctor {input_select_8}

Some of my fields are required too, so it's not like you can just go to the website and submit a blank form. Why would this be happening?
