I can't receive the cv file and image attachment

Dear Sirs

I am trying to set up an upload cv and image on a form.
The form send all the other details ok but I don't receive the attachments by email.

Please can you tell me what I should do to make this working?

What I did is:
Added the upload field on the preview window and the back end I am not sure what I need to tick..
Please see my attachment

Hi Veronica,

Please make sure that you have added and configured an "Upload files" action and that you have added the file field name to the "Attachments fields names" box inside your email action configuration.
