Static or dynamic?

Hi Bob, I was on and on trying to understand which fields do I need to use, but I'm very confused!
So, could you tell me please exactly which fields should I need?
This the scenario I'd like to have:

When a user completes form in the web site, I'd like to receive it this way:
From name: dynamic name from the form
From e-mail: dynamic e-mail from the form
To: [email]info@mydomain.com[/email]
CC: [email]other@mydomain.com[/email]

When I reply the message :
To name: dynamic name from the form
To e-mail: dynamic e-mail from the form
From name: My domain
From e-mail: [email]info@mydomain.com[/email]

From name: dynamic name from the form - use the Dynamic From Name
From e-mail: dynamic e-mail from the form - use the Dynamic From Email
To: [email]info@mydomain.com[/email] - use the Static To Email
CC: [email]other@mydomain.com[/email] - use the Static CC Email

This will give you exactly what you asked for. And there is a high probability that your emails will be marked as spam and lost. If this happens use these settings:

From name: - use the Static From Name
From e-mail: - use the Static From Email
Reply To name: dynamic name from the form - use the Dynamic Reply To Name
Reply To e-mail: dynamic e-mail from the form - use the Dynamic Reply To Email
To: [email]info@mydomain.com[/email] - use the Static To Email
CC: [email]other@mydomain.com[/email] - use the Static CC Email
