simple question editing HTML question

Hello,

I had a form working perfectly, but then i decided to change one of my questions and change the order of the questions. as a result, the form looks like it works on my wesbite (ie. questions are in the right order and check boxes work, old question is removed...), but they are not correct in the email of the results i get. The email contains the old question, one question doesnt answer correctly, and the order is screwed up. can someone take a quick look at the html and help me out? attached are screen shots of the email i get and the way the form looks on the website.

also, while im asking questions, is there a good way to export the results of the form to an excel spreadsheet? (feel free to ignore this question, because im sure i can just find a good answer by searching the forum).
