Layout problem with chronoform [SOLVED]

Hello 😀 ,
I have a problem with displaying a form chronoform from two different links.

This is the correct view:

while here the layout is all wrong:

http://www.asteconsulting.com/contratti-servizi.html

I have created the first module with no problems, then I duplicated and assigned to the second link and got this result. Any idea how to solve?

I'm not clear what the problem is when you say "The layout is all wrong"?

The 'Procedi' button is lower down on the second example, but you have 6 radio buttons there and 5 in the other form.

In fact i solved the the problem yesterday, was not the form that did not work but was the assignment of modules to be wrong. Now we can see the page correctly.
