Price list form

Hi,
.I have a new post ...I need to make a list of services offered, with attached prices and radio buttons , so the user will be able to click the radio buttons and chose the services s/he needs. This list of services will be part of the registration form ( I already created and is working just fine ) ...but I would like to be able to capture the info about what services is the user interested in, along with the general user's info...I hope isn't too confusing !
Any advice ...?

Create a table and save the data with a DB Connection.
