the form shows up fine on my iphone.. but n a balckberry the field name overlaps the text area... any suggestions?

I think that the answer is to create a separate style-sheet for mobie devices. You'll probably need to put the labels over the fields and maybe reduce some of the field widths. I've never tried a phone version but I guess it's a bit like re-formatting to use a form in a side column module.
