Set Up Emails Error

Am having a problem with Setting Up Emails

The top half of the attached shows what is seen after setting up the email
The bottom half shows what is seen after applying/saving the form

What's happening?
Correction ... what am I doing that is causing this and how do I fix it?

Hi Yerg,

You have to put values in all the boxes - input names for the Dynamic fields; text string/email addresses in the non-dynamic ones.
