Form contents not displaying in email

Hi,

I have successfully set up a form (correctly I believe) however the email of the form contents that is being sent to me is leaving the form areas where content should be blank. The email works however wehre content delimteres such as {Message} are, nothing is being printed so I just get the basic message template text. Any ideas as to what I have done wrong?

make sure you didn't rename fields after the template generated or edit it too
