Registration form fields

I am using J1.5 on PHP5 at http://www.globalteamdirect.net and I just created my first registration form. The problem I am having is once I completed the tutorials instructions in creating the registration form step by step I am not seeing the form when I click on Create An Account. It is still showing the default registration form.
The default registration form does not ask for First & Last name info so I created a field labeled "First and Last Name" and as I have a bit of programming experience but far from a pro I am not sure if I am suppose to go about linking the data fields to a DB table or how to if that is the case. Help!

OK a slight rephrase here...
I want the new registration form to use the existing users DB so if someone could walk me through that it would be appreciated.

Well I tried that plugin and entered the name of the form "Registration" into it and now I get Redirect Loop that will not resolve itself. I am using sh404sef and not sure how to correct this.

please disable SEF a moment and try to enter the Joomla registration page through the link in the Joomla login module, you get the redirect loop ?
