form link doesn't show

after i copied the code and hit save...the link to the form doesn't show...even after i published it.

help!

I think it came after that. It's an easy fix - the code change is in this thread.

I've also posted a patch to the current version which will include the change. But Max is working on a new release now so it might be worth waiting a few days before doing the full upgrade.
