Site Template Gone but Form Shows

Latest chronoforms install with latest plugin.... Created a menu link to the article (uncategorized) which has the plugin code {chronocontact}contact{/chronocontact} in place (and yes the plugin is active). Go to click on the Contact Us link on the front end and behold, the site's template is not there, "only" the form is showing.

Notes: Using sh404SEF but I tried this with the sef completely turned off...same thing. Also using Joomla 1.5.9.

Any ideas?

Never mind.... found the issue. I was parsing the image Submit button with php for Joomla so I made the submit button as a straight up absolute path.
