Cannot Save in Forms Manager after Upgrade

I just upgraded from v1.7 to v2.1. I uploaded all the new files manually through FTP to the components/com_chronocontact folder and to the administrator/components/com_chronocontact folder. My forms are still there, but when I try to make a change in the Forms Manager area, after I click Save, I just get a blank screen. The URL shown is http://www.mydomain.org/administrator/index2.php. I was not having any problems saving before the upgrade. Thank you for your time.

You will need to edit the table jos_chronocontact alittle since the table has got some changes too, please add those fields :

`onsubmitcodeb4` LONGTEXT NOT NULL

Can you please tell me where this new field gets inserted? Does it go at the end after the "published" field?

The MySQL command that you need to apply to the table is
ALTER 
TABLE `joomla`.`jos_chrono_contact` 
ADD COLUMN  `onsubmitcodeb4` LONGTEXT NOT NULL 
AFTER `onsubmitcode`;
