I would like to create a form with basic question of my guest users
Name
City
e-mail
and then i would like to have Check Items
I have a problem how to create the check Item. The check item is already exist in my joomla as com_content. I want automaticaly my form dynamicaly build based on my menu topic title of my joomla , is that possible ? And everytime i add new content in my site , will automatic update my form as new Check Item.

I think you are using Chronoforms here ? because this is ChronoConnectivity''s forums!
you will need a simple SQL query to load the menu items of your site and do some checks with it, do you have any PHP experience ?
