How to make this work?

My problem is next:
I would like to create form like this (this is just example):

The thing is that I would like that user who choose "Get free pizza" (radio button) may enter data only in fields (text box) below that option - "Name", "Surname", "Address" but NOT in the fields "Your email" and "Your favorite food".
Also when someone choose "Be on our maling list" to be able only to fill in fields "Your email" and "Your favorite food".

How to manage that?

Hi Mr one, this will need some Javascript code to be written by you to do this function, Chronoforms can't handle this out of the box, if you are working in the code and need a quick help then you can ask.
