I have tried myself to madness, but can not find the solution for the following attached form. always no data saved on the database table and no pdf file uploaded. Can you please help me out?

Form with dropdown selection and conditional multifields. Upload pdf and data to be sent to database. Confimation after completion.

the Upload directory path in the upload action is empty, you should provide one, and you can try to remove the upload action completely and use the global upload files setting, configure the file upload settings in the "File" field.

so to fix this, remove the "Upload files" action, and configure all the necessary settings in the "File" field, provide the extensions and other behaviors needed.
