I would like to create a form that has 2 columns
1 side label and the other side value.

Is there a video with instructions.

The purpose is to create a client profile with photo and attached documents.

There are many ways to do this but the easiest is to use a Grid Area. Navigate to Areas and select Grid. Drag the Grid onto your form. Change the Grid configuration by clicking the Edit icon in the upper right hand corner of the Grid panel. Next, click the blue Column button to create a second column and then press the orange Update button below. You should now have two columns below called 'row1_column1' and 'row1_column2'. Drag and drop whatever element you want to appear in each column into the appropriate 'row_column' area.
