Form Not Sending and How To Change URL After Submission

1) For some reason after I submit the form I don't get the email. I'm not sure what I'm missing.
The email in the form is using the site domain email.

2) How can I change the url that is showing after the form is sent.

This is what I see:
https://www.mysite.com/get-in-touch.html?chronoform=contact-form

This is what I would like:
https://www.mysite.com/get-in-touch.html

#1- does it work when you test in the admin area ? if yes then do you have cache enabled ?

#2- The form alias is added to the URL so that the form can find which form has been submitted, this is necessary, and has no impact on your SEO
