Email Template Data Missing

I seem to be having a problem getting Chronoforms to send me the custom email template data I have setup. I've created a simple email template which should work. I do get an email which only includes the IP address of the person who submitted the form. Below is what I have set for my email settings on my form. What's missing??

Joomla 1.5.7 (Chronoforms 3.0 Beta 2)
email result: Yes
email properties (enable): Yes

Email Template:

New Registration:
Last Name: {lastname}
First Name: {firstname}

Hi zoom, from the package in this post :

extract then get chronocontact.php and overwrite yours with the new one, it will fix it

or maybe you can reinstall the whole package if you still didn't do anything ? or backup then restore forms
