file upload information is missing

In my form with 2 upload files happen strange things.
​
Both files are attached to admin email as well as to custom email. The upload to the directory works fine for both files. Both entries in my custom table are there.
​
But in the admin email the filename is missing in mail content and it is also missing in database log.
​
Any ideas?
