Loosing Mails! Not all Mails are Send/Stored.

Hi,
I'm using the pruchased Version (6.1.4) of Chronoforms and got an strange Error.
Some Customers calling cause they used the inquiry Form and getting an send confirmation after submitting the Form .

But no Mail was received, I also storing all data in to the Database, but there isn't any Entry. After I looked into the Access log of the Apache2, there I can see that the customer really submitted the Form.
I also log all PHP Mail transactions, there isn't any Entry.
This happens not with all Form Submits, but this Year it happened 3 times.
Used PHP Version: 7.3.11

What the Hell is going on here?I'm loosing Money and Reputation.

Hello,

Only the form debug can tell you if the email was sent or not, if the email is not in the PHP mail log then it has failed.

If the record is not in the database too ( are you sure ? ) then some conflict might have occurred, this is not a known issue, does the Apache log have the form data sent ? you can try to the send the form again with the same data and see if the form works or fails, any 3rd party plugin or module on your website can cause the conflict.

Also please post a screenshot of your form setup (submit page actions)
