Getting a title on a page

OK.... so I clicked on the Form Code Tab and added a header there... it seems to be working. Hope that's the right way to do it.

if you have joomla 1.5 then you can create a page of type component and then choose your page title also, but your solution is just fine!

What do you mean by making a component page? You mean a menu link to the chronoforms component?
If you set the page title in there, it doesn't show up.

Try adding this to your form html:
