Blank form after submit

Dear all,

This is first time I got blank form after submit. So I change the error information to maximum and I got this message.

I need to include a filename.php in the custom code function. When im using include (JPATH_SITE.'/components/com_chronoforms5/includes/'.$form->form['Form']['title'].'/Net/SFTP.php'); I will get an error like this

Parse error: syntax error, unexpected end of file in C:\xampp\htdocs\projects\preast\administrator\components\com_chronoforms5\chronoforms\actions\custom_code\custom_code.php(20) : eval()'d code on line 31
