Ajax inconsistent in form as module

I have a form using Ajax to update a selection field. The Ajax works fine when the form is stand-alone, but when I assign the form to a module position, the Ajax only works sometimes. Wondering if there is some sort of cache setting I need to be careful of when using a form in a module position. Any suggestions are appreciated.

Did some more testing. The form-as-module works fine on all pages except the home page. The other pages I have it showing up on as a module all happen to be chronoconnectivity listings, if that makes any difference.

Hi Brian,

Are you using the ChronoForms module to show the form?

Are there any JavaScript errors showing on the pages where it doesn't work?

Yes, using the Chonoforms module to show the form. Getting this error on the console:
index.php:1 Uncaught SyntaxError: Unexpected token < in JSON at position 0

However, I am not getting this error when using the module form from any other page that it appears on. Just the home page.

Hi Brian,

I suspect that is not from ChronoForms, please post or PM a link to the home page and I'll take a quick look.
