Form Input Fields Are Cut Off

Hi,

I was testing my site today and noticed the input fields are cut off, in other words the user can not see the date they are entering. there have been no recent software updates on my end. doesn't seem to happen in v5. Seems to just started happening. Here's screenshot.

It looks to me like a CSS problem - Please post a link to the form so I can take a quick look.
