Strange behavior, never seen this before

I create the form, link a menu to it, the menu link works fine. when I submit I get a message saying
There is no form with this name or may be the form is unpublished, Please check the form and the url and the form management

also, in the form management, if I click the form url, I get the same message.. What is going on here???

any ideas?

Hey, turns out this was caused from the form name having a dash in it. I changed it to an underscore and all is working.. PHEW!!
