Access Denied with usergroup set

I have the usergroup sets on CCv5 but i still get an access denied message. It doesnt happen all of the time, the same login used in a different compuer/network and has access to it, this is also different forms, different websites all having the same issues. The only thing in common is that they are being used in an iFrame. Any thoughts?
