I have 8 forms on my site, every time a visitor submits the form, some of the required fields are missing from the email. However, if you fill out any of those fields incorrectly, you will get a warning stating so. Any ideas?

I think that this is probably just spam. I checked the form and it is possible to submit it empty if JavaScript is disabled (though it is practically invisible).

If you add some basic server-side validation I think that you will block most of these posts.

PS I suggest that you don't use an email address as a username here - it is publicly visible and may get scraped and abused.
