Currently I have the following setup:

The form is public form for our job seekers to fill out.
Upon submission, the fields are saved to the database, and an email is sent to the staff for the related county letting them know there is a form for review in the system.
Once that is submitted, staff members must go to the site, login to the system, http://mwforms.miworksmo.org/images/formimgs/dvop/login-screen.png at which time they are presented with a menu item "DVOP Referral" http://mwforms.miworksmo.org/images/formimgs/dvop/welcome-screen.png that points to a chronoconnectivity front end item http://mwforms.miworksmo.org/images/formimgs/dvop/DVOPReferrals-screen.png .
As of right now, when you click on "Edit" the form opens as such: http://mwforms.miworksmo.org/images/formimgs/dvop/DVOPReferrals-edit.png
and all fields are editable, and changes are made to the database...
What I would like to do is make everything above the "FOR MWA USE ONLY" read only, but still look like it presently does... and allow the bottom to be edited... I posed a question on here a week or two ago asking about how to make the fields read only, and I was told to put "readonly:readonly" into the "Extra Options" but the only thing I have is "Extra Params" in my objects... I did put the readonly:readonly into that field, but they are still allowed to edit all the data...
Please help me understand how to get this accomplished.

It should be readonly=readonly (not readonly:readonly), and yes, in the Extra params box.
Please try that and see if ti works OK

GreyHead, one last question, and this form can go to production... When the "edit" form gets completed, I would like to have an email sent out with a PDF of the documents, and I know how to get that accomplished on a form, but it isn't completing the action when I hit the submit button on the form that has the connection action in it... is this possible?
Hey GreyHead, I just realized that the radio buttons are still selectable with the form as well, is there any way to lock those down?

I'm sorry I don't understand the last question - which radio buttons and where? If they are set as readonly then they can't be clicked?

Disregard my radio button question, I converted them to txt boxes that are readonly.... but please attempt to address the question above it... I have reposted that here....
