I have the same problem, all fields are loaded unless the images and gives me the same error

Yes, I attached images with the sequence

The files names are stored in the database, and the files are uploaded, so all looks good!

Where is the problem ?

Now I checked that yes loads images. How do I retrieve data and images?

You can check the saved data by clicking the table's name in the forms manager, but if you want to load the data for some usage in your form then you can use a "db read" action or some custom query!
