@Greyhead, I know your going to cringe when you read this post but there must be an easy way to make this work. I'm just not rubbing my head and stomach in the right direction. I've tried all sorts of combinations in the static and dynamic forms to make this work but all logic eludes me. You caution of emails not being delivered because they get caught in a spam folder is outweighed by the clients desire to have the from information populated with the visitors name and email address.
Because the static value for "From name" & "From email" are required I cannot get around it.
What I have tried:
1. Downloaded and installed the CFv4 Email [GH] action - Although it told me the action was successfully processed after installing the zip file it did not change anything. One of the main thing I was looking for was the combination of the static and dynamic and the update stated "The Static and Dynamic tabs have been combined on a new Email Setup tab. "
2. I even implemented this fix. http://www.chronoengine.com/faqs/2588-problems-with-reply-to-email-addresses.html
I'm not sure what this means or how to use it: Note: If you are an experienced user and you need to use the dynamic From Email box you can enable the curly brackets by adding ##OK## to the box. I tried this: ##OK## {first_name} in the Static From Name input box. and didn't work. The values in mail settings (global configuration) are used indicating it does not like what I put into the box.
If I put static name and email address into the static input boxes the static info is used. Is there some way I can put the variable names {name} {email} into the static input boxes.
I have used name and email variable names in the dynamic from fields and they do not work at all.
I'm very frustrated, is there some procedural documentation that outlines this simple request for CF - 4.0 RC3.4.1 that I am missing?

UPDATE: The dynamic reply to email does work, but whatever I put into the dynamic relpy to name or email does not.

First question - are you actually using the Email [GH] action? - I'm not clear if that installed or not (as you say 'it didn't change anything')?
You should find a new Actions group called GH Custom actions where you can get the Email [GH] action. It is a completely separate action from the standard Email action and doesn’t change that.

Thank you for reading into my email and finding the clue 'it didn't change anything', that is why I left it. I'm sorry it was so obvious and I thank you for responding.
You were correct, I assumed the changes would have showed up in my current email action, my bad. I found the email GH action and created a new email action with it and it worked. I used ##OK## {first_name} {last_name} in the "From name" form field and ##OK## {email} in the "From email form field and it worked.
I have one more question, is the from item on the email supposed to have the ##OK## characters in it? When I look at my inbox it has "##OK## Mike" see attachment.

Again, my bad on the last question, I only needed to add the ##OK## in the "From email" form field and not the "From name" form field. The from name form field will take just curly bracket vars.

The reason for making the From Email more difficult is that making this dynamic more often that not causes the emails to be marked as spam and not delivered. Many mail servers and GMail in particular treat anything where the from email domain doesn't match the sending domain to be relayed spam and drop it without question. They don't check the From Name entry though so that can be anything you like.
