Captcha not showing

The captcha not showing next to new forms...

It worked on the localhost (WAMPSERVER), but when we uploaded it to the domain, the captcha image disappears & a small cross appears where the image should be.

www.xtremealu.co.za (the contact form still have the captcha - we removed captcha for now from the Request a Quote page)

* Captcha code is in the HTML
* Disabled the SEF, but no change... (so we reactivated it)
* Test form also not showing captcha

Couldn't find the LOAD ACTION options - but once I changed the FORM from EASY to DEFAULT in the Form Manager, the ACTIONS options appeared for each form. Where able to click the LOAD CAPTCHA IMAGE ...
