A previous client contacted me concerning their Contact Form.
When you type the verification (CAPTCHA) code, it states that the code is incorrect when you submit.
Find attached the printscreens of the code typed & the error after submitting. Not sure if this error appeared from the beginning when the website was created (end May 2013) or now....

Is caching enabled on the form page? That will cause this problem.

The cache is OFF in Global Configurations. Is there another place it can be enabled (where I can disable it)?

I'm sorry but I can't work out what the problem is here. If you add &tmpl=component to the URL to remove the template then the Captcha appears to work correctly. I also built a little test form and that works OK too with or without the template.
I checked for Redirect settings and for Plug-ins that might be changing the data and don't see any that look problematic.
I also looked at the form settings and those appear OK

PS I did see that the third bigger form on the list can't be edited and I think that is a problem with the PHP max_input_vars setting being 1000.

I tested the form on my side with no problems. When I asked the owner of the site to check the form again, she said it still does the same. Can it be on their computer settings/security/etc?
Also, she now states that the first dropdown option is not "selectable". See attachment...
Not sure why this wasn't mentioned before (when the website first went alive)...

You might ask what browser they are using.
I checked the drop-down in Chrome and IE11 (in IE7, 8 and 9 modes) and don't see any problems with selecting the dropdown. Is it a problem of selection - or that it doesn't validate correctly?
If it is validation please try adding a default 'Please select' entry in the element Show Empty box.

To remove problems with different browsers, I added the PLEASE SELECT as the empty value (thanks)...
I've tested the form on all 4 browsers without any problems with the CAPTCHA - waiting for her to let me know what version browser she uses (maybe the CAPTCHA problem is because she is using an old version)...

The email from the owner of the site:
"Good Morning.
I use Google chrome. Ok, I tested it this morning. It worked fine in Internet Explorer, but it still gives an error message in Chrome for the verification code."
The version she uses are: "Version 36.0.1985.143 m - it states that it is up to date."
Any suggestions? Think I should redo the whole website in Joomla 3?

That's pretty recent (I'm on a 37 beta here as of a few hours ago). There has to be something else going on that stops her form submitting but I don't know what that is. Try a little test form with a HoneyPot and see if that works any better for her.

There shouldn't be any problem with CFv4.
How do you have the Check Captcha action configured? What actions are in the On Success and On Fail events?
