As you can see in the screenshot I set the data field drop down but nothing is displayed, where am I wrong?

The options look OK to me :-(
Please take a Form Backup using the icon in the Forms Manager and post it here (as a zipped file) or PM or email it to me and I'll take a closer look.

Hi Stefan,
Please set Enable to 'No' on the Dynamic Data tab of the Select Drop-down element. That's only needed if you are loading the options from a database table (or from some other source than the Options box).
