Form not showing

Hi, i seem to be having a problem trying to get my form to show up. I had chronoforms V4 installed and the form was working fine but i needed to add multiple columns hence I upgraded to V5. I managed to replicate the same form but for some reason the form does not show up on my site even though I have added the HTML Render Form on Load status but i can see the chrono credits being shown on the page.

I have even tried one of the demo forms but still cant see form, any ideas where i am going wrong?

Can you see the form from the Test Form icon in the Form editor, or the View Form link in the Forms Manager?

Thanks Bob for your response. Unfortunately not the form do not show up even when I click on Test Form, just shows the chronocredits. I have checked this using Firebug and the fields and code is all there but doesnt show up...
