If you look at the form without the template the datepicker is OK. The layout problems are probably from your template CSS. This FAQ may help.
You also have a JavaScript error on the page which may be because there is some kind of conflict over the definition of the Browser object :-( I haven't seen this before and I'm not sure what is causing it.
