Placeholders in more than one form

Hello again,

I bought and used the Placeholder Labels Tutorial for v4.

It works great, except if I have more than one form on a page. One of the forms takes the placeholders and the other form doesn't.

I tried changing the field names and ids, so now they are all unique.

Then I tried numbering the my_input values for each form; my_input1, my_input2, etc.

No dice.

Do you have any suggestions to help me get all my forms to look the same?
