404 - Category not found Chronoforms 4.01 and Joomla 2.5

I installed 4.0.1 in my Joomla 2.5.14 site. I built my form and can test it, see it via the Frontend view link in the Chronoforms manager.

However, when I attempt to run the form via putting it as a menu item, I'm getting a 404 error.

Please help on this.
