My Tooltip text is displaying as the label instead for the tooltip text. also is there away to make the tooltip beside the Label and not below the "section"

When I hover over the ToolTip it displays the text for the label and not the Tooltip text.
Attached is an image with rollover. (Tooltip should read "Include any additional information needed for invoicing purposes such as a Purchase Order or reference number.")
Also note the placement of the icon, I would like it to display beside the "label", I have more then one tooltip on the page so placing it with position in CSS will not work.
