menu items are gone on form page

I created two forms and used the default beez20 default template designated in the menu item for each form. One form has the page menu tabs across the top like all the pages, but one form does not and I can't find any differences in how I created them.
Any one know what would cause this?

I have the answer to this, so thought I'd share.
In Module Manager go the the menu you are using, in my case is was Top.
Module Assignment should be set to "On All Pages".
This fixed it so that all the pages reflected the top menu tabs.
