Form no longer works, but only when displayed by plugin

I'm having trouble with my email form. It worked fine in the past, but now it has become flakey.

The form is embedded in an article with the plugin. When submitting the form from here, 9 times out of 10 the form simply refreshes, clears the text fields, doesn't go to the Thanks Message, and never sends the email:

On the other hand, if I link to the form directly, it works just fine:

Any insight as to what could be causing this issue would be greatly appreciated.

This look like a problem with SEF URLs, the component name is missing from the URL

http://www.greenmarkit.com/contact?chronoform=contact&event=submit

. That might cause the problem

Please try setting Relative URLs to No on the form General tab (click the Form name link in the Forms Manager to open it).

I upgraded to the latest RC (was on 3.5, upgraded to 3.5.2). I also changed the setting you mentioned and cleared all of the Joomla cache to be on the safe side.
