Checkbox not recording data

I'm having trouble getting the form to record the checkbox results in the table.
It didn't seem to be a problem when testing the form, but now that I have it on the page it is. It records the other inputs, just not the checkboxes.

this is not working with IE vrs 8. I have tested it in Firefox and all is well. Any suggestions???

Please try turning off Input Formatting in your template Advanced Options tab.

Forgive me I am a novice. Is this in the form set up or elsewhere in Joomla? Either way I'm not locating that setting and a bit more direction would be helpful. Thank you in advance!🙂
