It looks as though the ChronoForms CSS files aren't being loaded.
Please post a link to the form so I can take a quick look.

Okay this is weird, I've uploaded the files + database to a webserver and the form now works as expected. The layout I've shown was on my laptop's localhost. So I only have problems locally. I've compared the two pages and frontforms.css isn't loaded locally.
One other thing, on another computer (with a different mamp installl) the form also didn't work locally.
Any idea what I could check to make it work locally as well?

You might check the live_site setting in the site configuration. If there is a value set try removing it.

Thanks for your suggestion, but the live_site string was already empty. I also checked the logs, nothing there. Very strange this, all my other components just work.

Sorry no bright ideas. You can use your browser web developer tools to check if the files is being loaded and if it is whether the content is corrupted.

Okay now I have the same problem on the live site. The temp css/php file of chronoforms is empty for some reason. If a admin could look into this, it would be much appreciated.

Please try turning off the Rok Zipper plug-in. It doesn't work correctly with the the ChronoForms files and I suspect that this is the problem.
