Help with emailing details

Hi There

I have created a form using chrono forms, using HTML Code to design the form, but when I try to send the email with the input's, I can't get the values customers inputed on form

How should I display it into email?

I have created a form, then in Events, added a Custom Code and then inserted my code into there

How can I display my custom code information into the email?
