Form suddenly doesn't work

This has happened three times and each time I have reset the db or created a new one. I have made no other changes to the site. All of my tests work, Everything was saved to the database and I receive the email with all of the correct info. Then, a while later (or the next day), I receive an email with no dynamic info and an empty record in the database. Please help before I pull all my hair out!! Here is all the vital info:

EDIT: I have been searching your forums and now think it may be Spambots. In one thread you mention "Hidden honeypots" to help combat this. How do I add a hidden honeypot to my form?

I too am curious about hidden honeypots. Does anyone have a technique of how to implement this that they'd like to share in exchange for great appreciation?
