Std HTML Form with Unique Dropdowns

Hi.
I have a standard HTML form with unique dropdowns for product registration. Two issues:
1. The data is not being captured in the database for the dropdown information choosen.

2. I want to send an email to internal customer service and also the user (for their records). The emails are not being sent however.

Hard to tell without seeing the form. Please take a Form Backup and post it here or mail it to the address in my sig.

Most likely you need to set the select name as an array. And there is a FAQ on email problems that you can read.
